How we work with you
Following referral:
- We undertake a needs assessment and risk assessment by a qualified risk assessor.
- We introduce the Clarendon Home Care worker to the service user and start service delivery immediately.
- We prepare a detailed care plan which is sent within three days of the initial visit, for signing by the service user or a relative.
- We provide the service user with a care book and service user guide.
- We update social services as often as agreed through care plan reviews and through a formal annual review attended by a Clarendon Home Care manager or Senior Home Care Assistant.
